Note: Within the first five days of making your claim and initial baggage loss, please contact the local airport where you placed your baggage claim. After five days of checked baggage loss, please download the Baggage Statement of Loss Form shown below. Please note that this form must be mailed within 14 days of travel to be considered for loss payment.

Please note: This form can only be used when Ravn Alaska was your final carrier. If you made connections to other airlines throughout your journey a claim must be filed directly with the carrier that you last flew on to your final destination.
